Job summary

Encompass Health in Shreveport, LA is seeking a seasoned Case Management Director to lead our hospital-based Case Management team and drive exemplary patient care outcomes. You will oversee the plan of care, discharge planning, and census management while guiding staff and ensuring regulatory compliance.

The role requires a BSN or relevant degree, active licensure, and preferred CCM/ACM certification. You will collaborate with insurers and healthcare partners to optimize care and manage

Qualifications

• Current CCM or ACMTM certification is preferred.
• Must be able to independently assess within the scope of practice.
• Experience in hospital-based Case Management and discharge planning preferred.

Responsibilities

• Oversee day-to-day operations of the Case Management department.
• Direct interdisciplinary plan of care, discharge planning, and census management.
• Act as patient/family advocate ensuring services meet needs.
• Provide guidance and training to Case Managers and staff on caseloads and regulations.
• Build relationships with insurers, self-insured employers, and healthcare networks.
• Recognize and celebrate staff achievements.
