The ideal candidate will have extensive leadership experience and be responsible for overseeing all operational aspects of the case management department, ensuring compliance and performance improvement.

This full‑time position offers a competitive salary range of $135,004 - $202,555 annually, along with the possibility of a sign-on bonus. Candidates must have a Bachelor's degree and experience in acute care Hospital Case Management.

Qualifications

• Bachelor's degree required.
• Master's degree preferred.
• Registered Nurse (RN) licensure, LCSW, or LMSW required.
• 3+ years' experience in acute care Hospital Case Management required.
• 2+ years' experience in case management leadership required.

Responsibilities

• Oversee all operational aspects of the case management department.
• Ensure compliance with care coordination and discharge planning processes.
• Use operational data to identify performance gaps and implement improvements.
