Job summary

Saint Mary’s Health Network in Reno is hiring a Director of Case Management to lead a comprehensive program and ensure quality patient care. The role oversees case managers, social workers, discharge planners, and utilization staff, guiding service design to align with the hospital mission.

You will drive staff development, coordinate services using continuous quality improvement tools, and ensure effective assessment and planning to meet patient and family needs.

Qualifications

• Licensed clinician in your state.
• Minimum 5 years post-graduate of an accredited school of Social Work for Licensed Clinical Social Worker.
• Minimum 5 years’ experience in a Case Management position.
• Analytical ability for problem identification and data evaluation.
• Basic to intermediate computer skills.

Responsibilities

• Develop staff and systems to operate a comprehensive Case Management Program.
• Provide leadership and supervision to case managers, social workers and discharge planners.
• Assess needs and plan services aligned with hospital mission and patient/family needs.
• Coordinate services using continuous quality improvement tools.
