Job summary

Saint Mary of Nazareth Hospital in Chicago, IL seeks a Director of Case Management to lead and develop the case management program. You will supervise case managers, social workers, discharge planners, and utilization staff, and design services aligned with the hospital mission and patient needs.

Responsibilities include assessing needs, planning care, coordinating services, and driving continuous quality improvement.

Qualifications

• Licensed clinician licensure in state required.
• Minimum 5 years’ post graduate of an accredited School of Social Work for LCSW.
• Minimum 5 years’ experience in a Case Management position.
• Analytical ability for problem identification and data evaluation.
• Basic to intermediate computer skills required.

Responsibilities

• Develop staff and systems to operate a comprehensive Case Management Program.
• Provide leadership and supervision to case managers, social workers, discharge planners, and utilization staff.
• Assess needs, plan, and communicate services aligned with hospital mission and patient/family needs.
• Coordinate services using continuous quality improvement tools.
