Job summary

Saint Mary’s Health Network, a member of Prime Healthcare, is seeking a dedicated LPN to coordinate Case Management activities in our Days shift. You will assist with discharge plans, coordinate referrals and authorizations, and help prepare Medicaid submissions.

Qualifications

• Knowledge of discharge planning/utilization management/case management terminology and functions.
• Active state LPN license required upon hire and maintain current.
• Experience with basic to intermediate computer skills.
• Current BCLS (AHA) certificate required upon hire and maintain current.

Responsibilities

• Coordinate activities of the Case Management Department under the direction of the Case Manager/Social Worker.
• Assist with development and implementation of discharge plans.
• Assist with case management referrals and authorizations.
• Maintain Medicaid state submission forms and file they per facility procedure.
• Assist with discharge planning by interviewing patients/families and gathering data to develop a safe patient-focused discharge plan.

Responsibilities

Responsible for the coordination of the various activities of the Case Management Department under the direction of the assigned Case Manager/Social Worker assist with development and implementations of discharge plans. Maintains and coordinates Medicaid state submission forms for presentation to on-site reviewer and files them accordingly or as per facility’s procedure. Assists with Discharge Planning as assigned by interviewing patients/families and gathering data to develop a safe patient-focused Discharge Plan.

Qualifications

EDUCATION, EXPERIENCE, TRAINING

1. Knowledge of Discharge Planning/ Utilization Management / Case Management terminology and functions, in both managed care and non-managed care environments. 2. Active state LPN license required upon hire and maintain current. 3. Experience in basic to intermediate computer skills/knowledge 4. Current BCLS (AHA) certificate required upon hire and maintain current.
