Job summary

Saint Mary’s Health Network in Reno, NV seeks a dedicated LPN to support the Case Management Department. You will coordinate discharge planning activities, assist with referrals and authorizations, and maintain Medicaid submission forms for review.

You will help develop safe, patient-focused discharge plans with input from patients and families.

Qualifications

• Knowledge of Discharge Planning/Utilization Management/Case Management terminology and functions.
• Active state LPN license required upon hire and maintain current.
• Experience in basic to intermediate computer skills/knowledge.
• Current BCLS (AHA) certificate required upon hire and maintain current.

Responsibilities

• Coordinate the activities of the Case Management Department under the direction of the assigned Case Manager/Social Worker.
• Assist with development and implementations of discharge plans.
• Assist with case management referrals and authorizations.
• Maintain Medicaid state submission forms for presentation to on-site reviewer and file them accordingly.
• Assist with Discharge Planning by interviewing patients/families and gathering data to develop a safe patient-focused Discharge Plan.

• Active state LPN license required upon hire and maintain current.
• Experience in basic to intermediate computer skills/knowledge
• Current BCLS (AHA) certificate required upon hire and maintain current.
