Jobot is seeking a Director of Case Management to oversee all hospital-based case management operations at a 50-bed inpatient rehabilitation facility. Lead, mentor, and develop a team of Case Managers and support staff, while guiding utilization review, discharge planning, and care coordination to improve patient outcomes and streamline transitions.

Reporting directly to the hospital CEO, this role offers relocation assistance, a competitive salary of $90,000 to $105,000 with quarterly bonus

Responsibilities

• Direct all Case Management department operations for a 50-bed inpatient rehabilitation hospital
• Lead, mentor, and develop a team of Case Managers and Case Management support staff
• Oversee utilization review, discharge planning, care coordination, and patient progression activities
• Ensure compliance with hospital policies, regulatory standards, and best practices
• Develop strategies to maximize quality outcomes and efficient patient transitions of care
• Monitor departmental performance metrics and implement process improvements
• Collaborate closely with physicians, nursing leadership, therapists, and interdisciplinary teams
• Support case management staff during high-volume periods and assist with patient coverage when needed
• Foster a positive team culture focused on accountability, patient experience, and operational excellence
• Work directly with executive leadership to achieve organizational goals and performance objectives
• Support certification and professional development initiatives within the department
