Job summary

Responsibilities

• Lead all Case Management department operations within the hospital.
• Oversee utilization review, discharge planning, and care coordination activities.
• Manage and mentor a team of Case Managers.
• Recruit, onboard, and develop new Case Management staff.
• Support improvements in patient experience and discharge planning outcomes.
• Collaborate with physicians, nursing leaders, therapists, and interdisciplinary teams.
• Monitor departmental performance metrics and implement process improvements.
• Ensure compliance with regulatory and accreditation requirements.
• Assist with utilization management initiatives and resource optimization.
• Carry a patient caseload as needed while supporting department operations.
• Foster a positive culture and high-performing Case Management environment.
• Partner with executive leadership to achieve organizational goals.
