Job summary

Jobot is seeking a Director of Case Management to lead all hospital-based case management operations at a 50-bed inpatient rehabilitation facility in Parkersburg, WV. This on-site role reports to the hospital CEO and focuses on discharge planning, utilization review, and care coordination.

You will mentor Case Managers, drive performance improvements, ensure policy compliance, and collaborate with physicians, nursing leadership, and therapists.

Qualifications

• Experience directing case management in inpatient hospital settings.
• Ability to lead and develop a Case Management team.
• CCM or ACM certification supported or encouraged.

Responsibilities

• Direct all Case Management department operations for a 50-bed inpatient rehabilitation hospital.
• Lead, mentor, and develop a team of Case Managers and Case Management support staff.
• Oversee utilization review, discharge planning, care coordination, and patient progression activities.
• Ensure compliance with hospital policies, regulatory standards, and best practices.
• Develop strategies to maximize quality outcomes and efficient patient transitions of care.
• Monitor departmental performance metrics and implement process improvements.
• Collaborate closely with physicians, nursing leadership, therapists, and interdisciplinary teams.
• Support case management staff during high-volume periods and assist with patient coverage when needed.
• Foster a positive team culture focused on accountability, patient experience, and operational excellence.
• Work directly with executive leadership to achieve organizational goals and performance objectives.
• Support certification and professional development initiatives within the department.
