Job summary

Dignity Health - St. Mary Medical Center in Long Beach, CA seeks a Director of Care Coordination to lead care coordination and social services, directing staffing, performance improvement, and reporting across the Medical Center and system.

You will partner with departments, physicians and post-acute providers to optimize patient transitions, mentor teams, and educate staff to raise performance and reduce length of stay.

Qualifications

• Bachelor's Degree in Nursing (BSN) or Masters in Social Work (MSW) or equivalent education and experience required.
• Minimum of 5 years of clinical care coordination experience or healthcare management experience required.
• Extensive operational experience, program planning, implementation, staff development, and needs assessment in healthcare environment required.

Responsibilities

• Direct and manage care coordination and social services.
• Improve LOS reduction and reporting needs within the Medical Center and Dignity Health.
• Collaborate with departments, physicians, administration, and post-acute providers to optimize patient transitions.
• Educate staff and lead performance improvement initiatives.
