Job summary

Legacy Community Health in Houston is seeking a Director – Medication Adherence & Access Team to lead programs improving medication adherence and access for patients with chronic conditions. This role oversees initiatives, collaborates with providers, and drives data-driven outreach to at-risk clients.

The position offers a Monday–Friday schedule, direct impact on community health, and opportunities for professional growth within a large FQHC network.

Qualifications

• Pharm.D or B.S. with equivalent experience.
• Current Texas Pharmacist licensure in good standing.
• Certification in immunization delivery, smoking cessation, and naloxone.
• Post-graduate pharmacy residency required; 5+ years healthcare experience.
• 1+ year of community pharmacy experience preferred.
• 3+ years of medication adherence counseling experience preferred.
• 1+ year Medicare market experience preferred.
• Experience in HIV/AIDS, geriatrics, and/or pediatrics preferred.
• Computer experience Windows, Excel, and accounting software.
• Must be able to manage multiple deadlines and decision making responsibilities.
• Adept in decision making responsibilities.

Responsibilities

• Provide oversight to programs that enhance medication adherence and access.
• Collaborate with a compassionate team to support patients in managing their medication regimens.
• Engage in data collection, communication with healthcare providers, and outreach to at-risk clients.
• Experience growth opportunities and support our mission to uplift the communities we serve.
• Oversee program development and implement improvements to ensure timely medication access.
• Participate in Medication Therapy Management for patients with chronic diseases.
