Job summary

The Baltimore City Health Department is seeking a Local Health Improvement Coalition (LHIC) Director to lead a 4-person team and coordinate cross‑sector efforts for diabetes prevention, care coordination, and social determinants of health.

Reporting to the Chief Policy and Strategy Officer, the LHIC Director will guide coalition activities, implement evidence‑based initiatives, and foster partnerships across government, community organizations, and healthcare providers to improve Baltimore

Qualifications

• Master’s degree in public health, health administration, public policy, public administration, community development, or related field.

Responsibilities

• Lead strategic planning and oversight of LHIC initiatives including health priorities like diabetes and social determinants of health.
• Cultivate partnerships with local government, healthcare providers, and community organizations; convene monthly coalition meetings.
• Develop outreach strategies to raise awareness of health priorities and engage priority populations.
• Oversee grant management, budgeting, and annual program planning; ensure compliance with grant guidelines.
• Support policy initiatives to promote health equity and coordinate with the Policy Office.
• Guide contract management with subgrantees and partners, ensuring timelines and scopes of work are met.
