Job summary

Baltimore City Health Department is seeking a Health Project Director (NCS) to lead the Local Health Improvement Coalition and guide strategic initiatives across Baltimore City. The role reports to the Chief Policy and Strategy Officer and oversees a multidisciplinary team focused on diabetes, care coordination, and social determinants of health.

The LHIC Director will drive partnerships, coordinate community outreach, and implement evidence-based programs.

Qualifications

• Master’s degree (or equivalent experience) in a relevant field.
• 3–5 years of experience in community health, coalition-building, or related areas.
• Experience in grant writing, fund raising, and budget management.

Responsibilities

• Lead the Local Health Improvement Coalition (LHIC) and oversee multiple community health projects.
• Cultivate and maintain partnerships with local government, healthcare providers, and community organizations; convene monthly coalition meetings.
• Oversee a 4-person team (1 manager, 1 coordinator, 2 epidemiologists) and drive strategy and evaluation.
• Coordinate Community Health Needs Assessments and Community Health Improvement Plan processes every 3–5 years.
