• Licensed clinical background (RN, LCSW, or related) with acute care experience preferred.

Responsibilities

• Oversee and lead the hospital's case management functions on-site.
• Coordinate interdisciplinary discharge planning and patient transitions.
• Collaborate with physicians, nursing, social services, and other departments to maintain continuity of care.
• Monitor utilization management activities, patient status, and length of stay; ensure payer requirements are met.
• Develop, update, and implement case management policies; educate staff and promote process improvements.
