Qualifications

• Analytical ability for problem identification and data evaluation.
• Experience and knowledge of basic to intermediate computer skills.

Responsibilities

• Develop staff and systems to operate a comprehensive Case Management Program.
• Provide leadership and supervision to case managers, social workers, coordinators, and utilization staff.
• Assess needs, plan, communicate, and design services aligned with hospital mission and patient/family needs.
• Coordinate services using continuous quality improvement tools.
