Job summary

CHI Health St. Elizabeth in Lincoln, NE seeks a Director of Care Management to provide strategic leadership and comprehensive operational oversight for care management programs across the continuum.

You will lead multidisciplinary teams, develop and implement plans to optimize patient flow, and ensure high-value, patient-centered care. You will oversee regulatory compliance, quality metrics, and build strong relationships with physicians, payers, and post-acute providers, driving innovation

Qualifications

• Bachelor's degree is required.
• Licenses/certifications as listed are required.
• Mandatory Reporter certification within 6 months if working in Iowa or behavioral health unit.
• Excellent customer service and presentation skills.
• Strong interpersonal and written communication skills.
• Analytical and problem-solving ability.
• Ability to manage multiple tasks/projects.
• Ability to work independently with high attention to detail.
