Job summary

St. Mary Medical Center in Long Beach, California, part of Dignity Health, seeks a Director of Care Coordination to lead care coordination and social services, driving LOS reductions and regulatory reporting.

The role collaborates across departments and with post-acute providers to optimize patient transitions; requires an RN with a BSN or MSW and at least five years of leadership experience, CA RN or LCSW license, and CCM/ACM certification or within two years of hire.

Qualifications

  • Bachelor's Degree in Nursing (BSN) or Master's in Social Work (MSW) or equivalent education and experience required.
  • Minimum of 5 years of clinical care coordination experience (Utilization Management, Denial Management, Case Management, Care Coordination).
  • Extensive operational experience, program planning, implementation, staff development, and needs assessment in healthcare environment.
  • Comprehensive knowledge of care coordination and discharge planning required.
  • Current California RN license or California Licensed Clinical Social Worker (LCSW) required.
  • National certification in CCM or ACM required or within 2 years upon hire.

Responsibilities

  • Oversee care coordination and social services, directing all activities and performance improvement.
  • Streamline patient transitions through the health care system and post-discharge with community providers.
  • Collaborate with managers, physicians, administration, and community-based workers on care management issues.
  • Educate staff and communicate effectively, both one-on-one and in group settings.

Job Summary and Responsibilities

As our Director of Care Coordination for St. Mary Medical Center, you will be responsible for the overall direction and management of care coordination and social services including planning, organizing and directing all activities, staffing, performance improvement in the delivery of clinical services (such as LOS reduction) and reporting needs within the Medical Center and Dignity Health, as well as government and regulatory reporting.

Every day, you will work closely with all departments at the Medical Center and the post-acute service providers to streamline the patient transition through the health care system and into the community post discharge, and consult/collaborate with other managers, physicians, administration, and community based healthcare workers regarding care management issues identified through corporate or facility initiatives and current literature.

To be successful in this role, you must be competent and confident. Utilizing your expertise and excellent communication skills one-on-one and in front of groups, you will tackle tough conversations and, above all, be an educator at heart. Every interaction is an opportunity to share your knowledge with others and improve their understanding and performance, so that together we can optimize care coordination across the care continuum for our patients.

Our preferred candidate will be a Registered Nurse (RN) with a Bachelor's of Science in Nursing (BSN).

Job Requirements
  • Bachelor's Degree in Nursing (BSN) or Masters in Social Work (MSW) or equivalent education and experience required.
  • Minimum of 5 years of clinical care coordination experience (Utilization Management, Denial Management, Case Management, Care Coordination), or 5 years of progressively responsible healthcare management experience required.
  • Extensive operational experience, program planning, implementation, staff development, and needs assessment in healthcare environment required.
  • Comprehensive knowledge of care coordination and discharge planning required.
  • Current California Registered Nurse (RN) license, or California Licensed Clinical Social Worker (LCSW) required.
  • National certification in either of the following: CCM (Certified Case Manager), ACM (Accredited Case Manager) required orwithin 2 years upon hire.
