Job summary

Chandler Regional Medical Center is seeking a Care Coordination Manager to oversee daily operations of the care coordination and discharge planning functions, ensuring compliant, high-quality patient care across providers and community resources.

You'll lead staffing, performance improvement, and reporting, collaborating with physicians, administrators, and social work staff to drive timely care coordination and optimal patient outcomes at our 443-bed acute care hospital in the East Valley.

Qualifications

  • Bachelor's Degree in Nursing (or equivalent) or Master's in Social Work.
  • Three years of clinical care coordination or equivalent supervisory experience.
  • Operational experience in a healthcare environment.
  • AZ RN licensure or AZ Social Worker licensure.
  • ACM or CCM certification within 24 months.

Responsibilities

  • Provides operational leadership for the Care Coordination department through staff supervision, team development, coaching, and performance management.
  • Coordinates and implements Care Coordination programs, initiatives, and workflows to support patient progression, resource utilization, quality outcomes, and organizational goals.
  • Manages daily department operations, including staffing assignments, workload balancing, scheduling, productivity, and resource allocation; supervises front-line staff.
  • Ensures timely completion of patient assessments, discharge planning, utilization management collaboration, documentation, and care transition activities.
  • Monitors departmental performance metrics, identifies trends, implements corrective actions, and escalates significant issues to the Director.

Job description

As our Care Coordination Manager at St. Joseph's Hospital & Medical Center, you will drive the efficient and compliant oversight of daily operations for our case management functions, ensuring seamless coordination of care and services across all providers and community resources.


Every day you will manage all departmental activities, including staffing, performance improvement, and reporting, to meet all regulatory requirements, while consulting and collaborating with physicians, administrators, and community healthcare workers on care management initiatives.


To be successful in this role, you will leverage your leadership to ensure timely and compliant care coordination, discharge planning, utilization review, and social work interventions, ultimately resulting in the delivery of high-quality patient care and optimal outcomes


Job Responsibilities


  • Provides operational leadership for the Care Coordination department through staff supervision, team development, coaching, and performance management

  • Coordinates and implements Care Coordination programs, initiatives, and workflows to support patient progression, resource utilization, quality outcomes, and organizational goals

  • Manages daily department operations, including staffing assignments, workload balancing, scheduling, productivity, and resource allocation. Supervises front-line Care Coordination staff, including case managers, social workers, and support personnel, ensuring accountability for performance and service excellence

  • Ensures timely completion of patient assessments, discharge planning, utilization management collaboration, documentation, and care transition activities in accordance with organizational standards. Ensures compliance with federal, state, accreditation, payer, and organizational requirements related to Care Coordination and Discharge Planning

  • Monitors departmental performance metrics and operational data, identifies trends, implements corrective actions, and escalates significant issues to the Director. Implements departmental policies, procedures, standardized workflows, and process improvements to improve operational efficiency and patient outcomes


Job Requirements

Required


  • Bachelor's Degree in Nursing (e.g. BSN) or Masters in Social Work or equivalent education and experience

  • Three (3) years of clinical care coordination experience, (Utilization Management, Denial Management, Care Coordination) or 3 years of acute nursing team lead, charge or supervisory experience with working knowledge of care coordination and discharge planning

  • Operational experience, implementation and staff development in a healthcare environment

  • Registered Nurse: AZ or Licensed Masters Social Worker: AZ

  • Accredited Case Manager (ACM) or Certified Case Manager (CCM), within 24 months


Preferred


  • Master's degree in Nursing, Health Care Administration or related clinical field

  • Licensed Clinical Social Worker: AZ

  • Four (4) years of experience with performance improvement activities
