Job summary

Barnes-Jewish Hospital is seeking a Director of Care Coordination to lead IP/OP Social Work, Case Management, Spiritual Care and Rehab. This role directs patient care coordination across inpatient and outpatient settings to ensure timely discharges, optimal care transitions and alignment with hospital objectives.

The ideal candidate holds a Bachelor's Degree, 5–10 years of supervisory leadership, an RN license, and demonstrates budgeting, staffing, and performance management expertise to guide

• Manages onboarding and hiring, training, workload management and performance evaluations.
• Develops and adheres to departmental staffing, revenue and/or expense budgets.
• Monitors and evaluates resources in Case Management, Social Services and CHF Disease Management.
• Directs care coordination in the inpatient setting.
• Provides designated service lines and physicians with timely data and analyzes data for administrative focus.
• Oversees care of patients referred to Hospitalist Service to ensure timely discharge and communication with physicians, patients and families.

Role Purpose

Selects, orients, monitors, assigns projects, counsels, problem solves, leads employees under his/her jurisdiction. Practices in accordance with Missouri Nurse Practice Act and HSO policies and standards of care. Leads and directs the coordination of patient care at the most appropriate location, level and time.

Responsibilities

• Manages individual(s) including but not limited to: onboarding and hiring, training, workload management & performance evaluations, conducting professional development plans as needed. Ensures that the productivity and actions of that group meet/support the overall operational goals of the department as established by department leadership.
• Develops and adheres to departmental staffing, revenue and/or expense budgets. Responds to changes in the business which may affect the ability to achieve the budget goals.
• Monitors, controls and evaluates all resources utilized in Case Management, Social Services and Congestive Heart Failure Disease Management Departments.
• Directs the extension of care coordination in the inpatient setting.
• Provides designated service lines and physicians timely data, assists in analysis of the data and identifies areas for administrative focus.
• Oversees the care of patients referred to the Hospitalist Service to ensure timely hospital discharge and continuous communication with Primary Care Physicians, patients and families, specialists, hospital departments and administration.
