Job summary

Barnes-Jewish Hospital at Washington University Medical Center in Missouri seeks a Director of Care Coordination to oversee IP/OP Social Work, Case Management, Spiritual Care and Rehab, ensuring seamless patient care across inpatient and outpatient settings.

This leadership role directs staffing, budgeting, data analysis, and collaboration with physicians and departments to optimize discharge planning and care transitions.

Qualifications

• Bachelor's Degree is required.
• 5-10 years of experience in related field.
• 5-10 years of supervisor experience.
• RN licensure is required.

Responsibilities

• Onboards and hires staff, trains, manages workload and conducts performance evaluations.
• Develops and adheres to departmental staffing, revenue and/or expense budgets.
• Monitors resources in Case Management, Social Services and Congestive Heart Failure Disease Management.
• Directs the extension of care coordination in the inpatient setting.
• Provides designated service lines and physicians timely data, assists in analysis of the data and identifies areas for administrative focus.
• Oversees the care of patients referred to the Hospitalist Service to ensure timely hospital discharge and continuous communication with Primary Care Physicians, patients and families, specialists, hospital departments and administration.

Role Purpose

Selects, orients, monitors, assigns projects, counsels, problem solves, leads employees under his/her jurisdiction. Practices in accordance with Missouri Nurse Practice Act and HSO policies and standards of care. Leads and directs the coordination of patient care at the most appropriate location, level and time.

Responsibilities

• Manages individual(s) including but not limited to: onboarding and hiring, training, workload management & performance evaluations, conducting professional development plans as needed. Ensures that the productivity and actions of that group meet/support the overall operational goals of the department as established by department leadership.
• Develops and adheres to departmental staffing, revenue and/or expense budgets. Responds to changes in the business which may affect the ability to achieve the budget goals.
• Monitors, controls and evaluates all resources utilized in Case Management, Social Services and Congestive Heart Failure Disease Management Departments.
• Directs the extension of care coordination in the inpatient setting.
• Provides designated service lines and physicians timely data, assists in analysis of the data and identifies areas for administrative focus.
• Oversees the care of patients referred to the Hospitalist Service to ensure timely hospital discharge and continuous communication with Primary Care Physicians, patients and families, specialists, hospital departments and administration.
