Job summary

Qualifications
- High School Diploma or equivalent.
- Direct Service experience preferred.
- Experience with individuals with intellectual disabilities preferred.
Responsibilities
- Provide respectful, client-centered direct services and support following the treatment plan.
- Support community and daily living activities and connect client to resources.
- Monitor wellbeing, assist with personal care, and accompany to appointments.
- Ensure safety plans and compliance with regulations during community activities.
- Collect data, complete timesheets and documentation promptly.
- Collaborate with the team and advocate for the individual.
