Job summary

Responsibilities

• Assist individuals with daily living activities (bathing, dressing, grooming, toileting, eating).
• Support individuals in pursuing personal goals and community involvement.
• Administer medications and document accordingly (with proper training and certification).
• Monitor and report changes in health to supervisory staff.
• Maintain a clean, safe, and supportive environment.
• Assist with transportation to appointments or activities.
• Implement behavior support plans and follow interventions.
• Document progress, incidents, and daily activities per agency policies.
• Foster respectful, empathetic, and professional relationships with individuals, families, and team members.
• Attend team meetings, training sessions, and required certifications (CPR, First Aid, etc.).
