Job summary

Sevita is seeking a dedicated Direct Support Professional to provide hands-on assistance in residential settings. You will help with daily activities, health maintenance, and community integration under individual plans, ensuring dignity and safety for each client.

As part of the Sevita team, you will document progress, support medication administration if needed, and collaborate with families and case managers to maintain high quality care.

Qualifications

• High school diploma or equivalent preferred.
• Six months of experience in human services preferred.
• Must be 18 years of age.
• Working knowledge of computers.

Responsibilities

• Deliver support per individual service/program plans, including socialization, health maintenance, medication administration, skill/behavioral development.
• Assist with daily activities: Personal care, housekeeping, recreational activities, transportation, community integration, shopping, citizenship, and other activities of daily living.
• Accurately document progress and activity; review records and logs to stay abreast of changes in service plans; maintain confidentiality.
• Make cost‑effective and appropriate purchases within the budget of the individual served and according to their choice/preference.
• Maintain healthy and professional relationships with individuals, families, guardians, and case managers; greet visitors; uphold Sevita Customer Service Standards.
• Report any abuse/neglect and medical/behavioral incidents per policy; respect rights and follow safety procedures.
• Accompany individuals to medical appointments; relay orders to providers; monitor health and document concerns.
• Administer medications if assigned; ensure security of medications; report errors and follow procedures.
• Prepare or support meals according to planned menus and individual needs.

Skills
