Direct Support Professional – Periodic PT

We are hiring for a Direct Support Professional in a Periodic Part‑time role. The position provides support for individuals and groups of people with intellectual, developmental disabilities and those facing mental health and substance use challenges in residential and community settings. The role implements services and supports to address each person’s goals identified through their Individual Support Plan. The supervisor is the Community Services Coordinator.

Job Responsibilities

• Provide opportunities for choice and encourage clients to exercise control over themselves and their environment.
• Help clients identify personal outcomes that best suit their interests, cultural backgrounds and future dreams.
• Work with an interdisciplinary healthcare team to determine appropriate support methods to achieve stated goals.
• Implement services and supports that assist clients in achieving personal goals and desired outcomes outlined in an Individual Support Plan.
• Assist clients in the implementation of personal plans, goals and objectives and facilitate communication of needs and requests.
• Provide assistance and instruction on food preparation and house cleaning.
• Help clients identify and acquire valued social roles and develop healthy personal and social habits.
• Continuously assess functional levels, motivate clients and provide consistent reinforcement, assessment and feedback for all learning activities.
• Dispense medication, monitor dosages and make notes in charts per training level.
• Transport clients to and from day programs, work sites and community activities.
• Support clients at vocational sites to help meet responsibilities.
• Perform other duties as assigned.

Job Requirements

• High School Diploma or G.E.D.; Associate Degree in a health‑related area; or two years’ experience in a health‑related field.
• Experience working with individuals with developmental and intellectual disabilities preferred.
• Valid driver’s license and automotive insurance.
• Flexible schedule and availability for on‑call coverage.
• Certification in CPR, first aid and/or MedTech preferred.
• Complete pre‑employment screening: criminal background check, name check in registries (OIG exclusions database, Child Abuse Registry, Offenders Against Individuals with Developmental Disabilities), drug testing, education verification and employment references if required.
• Proof of driver’s license, insurance and vehicle if transportation duties are required.
