Job summary

Holcomb Behavioral Health Systems, a CHIMES company, seeks dedicated staff to provide support, training and quality care to residents in a community group home in Boothwyn, PA.

Under supervision from the House Manager, you will assist residents in reaching goals, serve as a positive role model, provide personal care, transport to appointments, and document activities, with a full-time night schedule.

Qualifications

• High school diploma or equivalent.
• Experience: None required.
• Licensure/Certifications: None Required.
• Required Clearances: PA Child Abuse, Criminal and fingerprint-based federal criminal history; Verification that employee is not on any Medicaid/Medicare Exclusion list.

Responsibilities

• Assist persons-served in reaching their goals of becoming more independent
• Act as a positive role model for persons served
• Provide guidance, instruction, coaching, and support, in accordance with individual plans
• Engage individuals in beneficial programs and activities
• Transport and accompany individuals into the community for appointments and outings
• Directly assist with personal care as needed
• Assist with meal preparation and routine homemaking duties
• Assume responsibility for the safety of the person served including medical/medication needs
• Apply approved behavior plans and intervention strategies as/if needed
• Complete records and reports; collect data according to Agency policy
