Job summary

Holcomb Behavioral Health Systems - A Chimes Company is seeking staff to support residents in a community group home as part of the Residential Habilitative Program. The role involves assisting residents with daily activities, personal care, and behavioral supports.

Shifts are part-time: Saturday-Sunday 4 pm–12 am; Monday & Friday 2 pm–12 am, in Boothwyn, PA. Education must include a high school diploma; prior experience is not required; several clearances are required.

Qualifications

• Education: High school diploma or equivalent.
• Experience: None required.
• Licensure/Certifications: None required.
• Required Clearances: Pennsylvania Child Abuse, Criminal and fingerprint-based federal criminal history; Verification that employee is not on any Medicaid/Medicare Exclusion list.
• Note: Additional related experience and/or education may be substituted instead of the requirements specified under Education and Experience.

Responsibilities

• Assist persons-served in reaching their goals of becoming more independent.
• Act as a positive role model for persons served.
• Provide guidance, instruction, coaching, and support, in accordance with individual plans.
• Engage individuals in beneficial programs and activities.
• Transport and accompany individuals into the community for appointments and outings.
• Directly assist with personal care as needed.
• Assist with meal preparation and routine homemaking duties.
• Assume responsibility for the safety of the person served including medical/medication needs.
• Apply approved behavior plans and intervention strategies as/if needed.
• Complete records and reports; collect data according to Agency policy.
