Job summary

Holcomb Behavioral Health Systems (a Chimes Company) is seeking caregivers to provide support, training, and quality care for residents with intellectual disabilities in a community group home located in Wallingford, PA. This role operates under supervision from a House Manager with a Division Manager overview.

Position is PRN with shifts to cover as needed. The role emphasizes independence, safety, personal care, and engagement in meaningful activities for individuals served.

Qualifications

• High school diploma or equivalent required.
• Experience: None required.
• Clearances: Pennsylvania Child Abuse, Criminal and fingerprint-based federal criminal history; not on Medicaid/Medicare Exclusion list.

Responsibilities

• Assist persons served in reaching their goals of becoming more independent
• Act as a positive role model for persons served
• Provide guidance, instruction, coaching, and support, in accordance with individual plans
• Engage individuals in beneficial programs and activities
• Transport and accompany individuals into the community for appointments and outings
• Directly assist with personal care as needed
• Assist with meal preparation and routine homemaking duties
• Assume responsibility for the safety of the person served including medical/medication needs
• Apply approved behavior plans and intervention strategies as/if needed
• Complete records and reports; collect data according to Agency policy
