Job summary

WellLife Network is seeking a Direct Support Professional to provide hands-on care and advocacy for individuals with developmental disabilities in their homes and communities in Rosedale, NY. You will assist with daily living, attend medical appointments, and help with community activities, while maintaining records in electronic systems.

Must have a High School diploma, English fluency, and a NYS Driver’s License; driving roles require it.

Qualifications

• High School diploma or equivalency diploma required.
• Fluent in English for client communication.
• Valid New York State Driver’s License with 1 year of driving experience is required; comfortable operating a large vehicle.

Responsibilities

• Provide hands-on care, encouragement, and advocacy for individuals.
• Assist with daily living activities and create a safe, comfortable home environment.
• Administer medications as directed and support medical appointments.
• Plan and participate in recreational, educational, and social activities.
• Maintain accurate records in electronic systems including daily notes and reports.
