The role emphasizes teamwork, ongoing training, and flexible scheduling to meet coverage needs; you may stay beyond shifts to ensure continuity of care and patient wellbeing.

Qualifications

• Must be at least 18 years old.
• High school diploma or GED.
• Valid driver's license and ability to transport individuals in agency vehicles.
• Able to complete required background checks and health screenings.
• Willingness to participate in required trainings and maintain certifications.
• Commitment to a safe, professional work environment and continuity of care.

Responsibilities

• Support individuals with daily living activities per care plans.
• Document health information, vital signs, and medication administration as trained.
• Transport individuals to appointments and community activities.
• Prepare meals and assist with nutrition and mealtime needs.
• Assist with mobility, transfers, and use of adaptive equipment as needed.
• Maintain accurate EHR documentation and comply with policies.
