Job summary

Bridgewell seeks a Direct Support Professional to assist people with developmental, physical, and psychiatric disabilities in residential and community settings. The role emphasizes daily living skills, supervision, and facilitating independence within a home environment.

Responsibilities include medication administration per MAP, documentation, and collaboration with families and agencies. A high school diploma or GED and relevant experience are required.

Qualifications

• Excellent written and oral communication skills.
• Excellent organizational and problem-solving skills.
• Computer literacy to include Microsoft Word, Excel, email, and web-based applications used by the agency.
• Ability to drive a large and/or wheelchair van with a valid US driver’s license and minimum one year of driving experience in the United States.

Responsibilities

• Provide support and assist people in our services with skills training in activities of daily living during day, evening and/or overnight hours.
• Create opportunities and provide support to people to develop independence.
• Administer medications according to MAP regulations, as applicable.
• Maintain daily progress notes, shift checklists, and documentation per regulatory standards and policy.
• Engage with guardians and stakeholders to support person-centered plans.
• Attend staff meetings and participate in supervision sessions.
