Job summary

Seven Hills Foundation is seeking a Direct Support Professional II in Leicester, MA, to empower individuals to lead independent lives in community settings. The role focuses on daily support, life-skills coaching, and health-related assistance within residential programs.

You’ll implement ISP and Behavior Support Plans, assist with medications, document progress, and mentor staff. Eligible candidates should hold a high school diploma, have a valid US driver’s license, and be at least 18 years

Qualifications

Must have a valid US driver’s license and be able to travel between locations.
Candidate must be at least 18 years old and able to meet the responsibilities of direct support.
Demonstrates empathy, patience, and strong communication in supportive roles.

Responsibilities

Provide daily support and care to individuals in a residential setting.
Help individuals build independence by teaching life skills at home and in the community.
Follow and implement Individual Support Plans (ISP) and Behavior Support Plans.
Assist with personal care, medication administration, and health-related needs.
Support individuals in community activities, relationships, and social connections.
Maintain accurate documentation and data collection to track progress.
