Qualifications

• Previous experience working with individuals with disabilities is preferred but not required.

Responsibilities

• Assist individuals with personal care and daily living activities.
• Support participation in community outings, recreational activities, and social events.
• Provide transportation to medical appointments, vocational programs, and community activities.
• Assist with meal preparation, household chores, and maintaining a safe living environment.
• Implement individualized service plans and supporting personal goals.
• Providing supervision and ensuring the health, safety, and well-being of individuals served.
• Completing required documentation, daily notes, and electronic records accurately and timely.
• Observing and reporting changes in physical, emotional, or behavioral health.
• Responding appropriately to emergencies and applying CPR/First Aid procedures when necessary.
• Maintaining confidentiality and adhering to HIPAA, DDS, and agency policies and procedures.
