Job summary

Holcomb Behavioral Health Systems is seeking a Residential Habilitative Program staff member in Wallingford, PA.

The role provides support, personal assistance, training, and quality care for individuals with intellectual disabilities living in a community group home.

Qualifications

• Education: High school diploma or equivalent.
• Experience: None required.
• Licensure/Certifications: None required.
• Required clearances: Pennsylvania Child Abuse, Criminal and fingerprint-based federal criminal history; not on Medicaid/Medicare Exclusion list.

Responsibilities

• Assist persons-served in reaching their goals of becoming more independent.
• Act as a positive role model for persons served.
• Provide guidance, instruction, coaching, and support, following individual plans.
• Engage individuals in beneficial programs and activities.
• Transport and accompany individuals into the community for appointments and outings.
• Directly assist with personal care as needed.
• Assist with meal preparation and routine homemaking duties.
• Ensure the safety of the person served including medical/medication needs.
• Apply approved behavior plans and intervention strategies as needed.
• Complete records and reports; collect data according to Agency policy.

Job Description

Under the general direction of the Division Manager of Residential Services, with specific supervision from the House Manager, is responsible for the provision of support, personal assistance, training, and quality care to people with intellectual disabilities residing in a community group home.

Schedule Details

Full-Time Shift

• Tuesday - Saturday 8:00 am - 4:00 pm

Job Duties
