Holcomb Behavioral Health Systems in Wallingford, PA is seeking staff for the Residential Habilitative Program to provide support, training, and quality care to individuals with intellectual disabilities living in a community group home.

Position involves assisting with personal care, meals, transportation, and executing individual plans; no prior experience required. Competitive benefits and full-time schedule offered. Located in Wallingford, PA.

Qualifications

• Education: High school diploma or equivalent.
• Experience: None required.
• Clearances: PA Child Abuse, Criminal and fingerprint-based federal criminal history; not on Medicaid/Medicare Exclusion list.
• Note: Additional related experience and/or education may be substituted at the discretion of the Personnel Officer.

Responsibilities

• Assist persons served in reaching goals of independence.
• Act as a positive role model for persons served.
• Provide guidance, instruction, coaching and support per individual plans.
• Engage individuals in beneficial programs and activities.
• Transport and accompany individuals to appointments and outings.
• Assist with personal care as needed.
• Assist with meal preparation and routine homemaking duties.
• Ensure safety and manage medical/medication needs as required.
• Apply approved behavior plans when needed.
• Complete records and reports per agency policy.

Job Description: Under the general direction of the Division Manager of Residential Services, with specific supervision from the House Manager, is responsible for the provision of support, personal assistance, training, and quality care to people with intellectual disabilities residing in a community group home.

Location: Wallingford, PA

Program: Residential Habilitative Program

Pay Range: $17.50/hour

Job Duties:

• Assist persons-served in reaching their goals of becoming more independent
• Act as a positive role model for persons served
• Provide guidance, instruction, coaching, and support, following individual plans
• Engage individuals in beneficial programs and activities
• Transport and accompany individuals into the community for appointments and outings
• Directly assist with personal care as needed
• Assist with meal preparation and routine homemaking duties
• Assume responsibility for the safety of the person served including medical/medication needs
• Apply approved behavior plans and intervention strategies as/if needed
• Complete records and reports; collect data according to Agency policy

Education: High school diploma or equivalent

Experience: None Required

Licensure/Certifications: None Required

Required Clearances: Pennsylvania Child Abuse, Criminal and fingerprint-based federal criminal history; Verification that employee is not on any Medicaid/Medicare Exclusion list

Note: At the discretion of the Personnel Officer, additional related experience and/or education may be substituted instead of the requirements specified under Education and Experience
