Out of the Box Solutions, LLC is seeking Direct Support Professionals to assist individuals with ID/DD in their homes and community settings. DSPs work one-on-one to follow individualized support plans, promote independence, and help with daily living, social activities, and safety.

Ideal candidates have a high school diploma, CPR/First Aid certification, reliable transportation, and a compassionate, dependable approach.

Formación

• High school diploma or equivalent required.
• CPR/First Aid certification required.

Responsabilidades

• Deliver training, support, and assistance to clients in personal care, recreation and leisure, daily living skills, social skills, health needs, personal preferences, exercising of individual rights, community integration, and behavior management.
• Assist in implementing and monitoring the clients' individualized support plan and tailor it based on observations.
• Complete required documentation including clocking in/out, weekly progress notes, and communication with families and office staff.
• Ensure clients health and safety at all times including prevention of abuse, CPR, and First Aid.
• Collaborate with client/family to meet and work toward goals.
• Assist with cooking, cleaning, medication, scheduling, appointments, shopping, and more.
• Support skill development as defined in the individual's plan and document progress notes.
