Qualifications

• Reliable transportation and a valid driver’s license are often required.

Responsibilities

• Provide one-on-one support in home and community settings.
• Assist with daily living activities such as cooking, cleaning, grooming and personal care.
• Encourage participation in social and community activities.
• Document care provided and progress.
• Communicate with the care team to ensure consistent support.
