Easterseals PORT Health in Zebulon, NC is seeking a Direct Support Professional to join our Group Home. This part-time role provides person-centered support to residents, assisting with daily living activities, mobility, personal care, transportation, and health care tasks.

You will follow Plan of Care, work with families and healthcare providers, document progress notes, and contribute to safety and quality of care.

Formación

• High School Diploma or Equivalent required.
• A valid driver’s license and auto insurance required.
• One year of documented experience with the population served preferred.
• Must be able to physically lift a minimum of 50 pounds.
• Must be able to speak audibly, write legibly, and perform repetitive motions.

Responsabilidades

• Provide person-centered support for activities of daily living, personal care, transportation, medication management, and community engagement to promote independence.
• Collaborate with individuals, families, supervisors, healthcare providers, and resources to support person-centered goals and safety.
• Coordinate and deliver direct support services including skill-building, behavior support, meal prep, and adherence to individual support plans.
• Document, monitor, and maintain progress notes, medication records, incident reports, and health observations for quality care standards.
• Support continuous improvement by collaborating with team members and participating in training and quality initiatives.
