Job summary

The Association for Individual Development (AID) is seeking a Direct Support Professional to implement individualized strategies for clients with disabilities, ensuring dignity, rights and safety while documenting daily activities across assigned sites. The role requires evenings/weekends schedule, transportation provision, and assistance with activities of daily living.

A High School diploma or GED is required, CNA or Autism Certificate is preferred, and driving is required.

Qualifications

• High School Diploma or GED required.
• CNA (Certified Nursing Assistant) or Autism Certificate desired.
• Experience working with developmentally disabled adults preferred.
• Must have and maintain Illinois driver’s license.

Responsibilities

• Maintaining a team approach in working with the clients, staff, and outside agencies.
• Implementing specific assigned programs from the Individual Plan or Implementation Strategy, and documentation per program procedures.
• Be familiar with and follow all agency and program rules and regulations. Reports concerns or issues to immediate supervisor without delay.
• Plan and prepare meals by following menus and recipes written by dietician with client input and assistance; adheres to client diet requirements.
• Attend to the self-care needs of the clients including toileting assistance, changing soiled garments, bathing and grooming, shaving, cleaning glasses and adaptive equipment, etc.
• Obtain medication administration authorization at a minimum of one location.
• Maintain medication administration authorization by attending annual training, following all policies and procedures, and minimizing preventable medication errors.
• Assure safe conditions inside and outside the home including maintenance and housekeeping.
• Must maintain a clean, sanitary environment, inside and out of all assigned buildings and vehicles.
• Provide transportation for clients in personal or agency vehicle for work, shopping, appointments, social events, etc.
• Must have knowledge of client’s medical conditions and be able to monitor and relay information to and from medical appointments.
• Attend all meetings, shifts, and trainings, as scheduled.
• Assist in the orientation/training of new staff.
• Participate in the planning and implementation of community and in-house activities.
• Take verbal and written directions and complete job responsibilities with minimal supervision.
