1250 Prader-Willi Homes of Oconomowoc LLC is seeking a Direct Support Professional to assist adults with Prader-Willi Syndrome in daily living, routines and community activities. You will support individuals with goal-oriented plans in a nurturing, structured environment designed for growth and safety.

The role emphasizes person-centered care, teamwork, and ongoing training to build skills and independence for residents.

Qualifications

• High school diploma or GED required.
• Experience with developmental disabilities or residential settings preferred but not required.
• Paid in-service training will be provided.

Responsibilities

• Provide supervision and support to residents in home and community settings.
• Assist with daily living activities (hygiene, personal care, meals).
• Support residents with ISPs and TBPs.
• Foster independence through structured routines and positive role-modeling.
• Facilitate structured activities based on residents' daily schedules and goals.
• Document progress, behaviors, and medical concerns clearly and promptly.
• Monitor health and safety; administer medications per procedures.
• Collaborate with team; attend meetings and contribute to treatment planning.
