Job summary

Makom is hiring Direct Support Professionals to help individuals with IDD lead self-determined, meaningful lives by engaging with their communities and pursuing a quality life.

You will provide personalized support, help develop skills, and promote empowerment within a supportive team. The role requires a high school diploma and strong communication, with physical ability to assist as needed.

Qualifications

• High school diploma required; further education in human services or related field is a plus.
• Excellent problem-solving and interpersonal skills; ability to work independently and with others.
• Active participation in cultivating a culture of communication with people we support, families, friends, and coworkers.
• Proficiency with Microsoft applications and general computer skills.
• Maintain professionalism and positive relationships with individuals, families, colleagues, and external partners.
• Ability to lift up to 100 pounds with assistance and kneel to provide hands-on support.

Responsibilities

• Person-Centered Support: Provide individualized support based on each person’s unique skills, aspirations, and needs.
• Skill Development: Support skill-building and personal growth per PCP/ISP.
• Community Engagement: Create opportunities to engage with communities and build social networks.
• Daily Living Assistance: Support ADLs and IADLs tailored to everyone’s needs.
• Health & Safety: Ensure clean, safe environments and follow care plans.
• Adaptability: Evolve approach as individuals’ needs evolve.
