Job summary

Minn Care Home Health LLC in New Hope, MN is seeking Direct Support Professionals for our Assisted Living program. You will provide direct client care per physician orders and the client's care plan, supporting activities of daily living and safety.

The role requires English fluency, empathy, a valid driver's license, and a passion for helping adults with developmental or mental health needs. Starting pay is $18/hr with benefits and community-based care.

Qualifications

• Must pass the Department of Human Services background check.
• Must be reliable and have a strong work ethic.
• Fluent in English and have good communication skills.
• Must show compassion and empathy to clients.
• Must have a driver's license.

Responsibilities

• Assist clients with personal care: dressing, grooming, bathing, eating, toileting, transferring, positioning, mobility.
• Housekeeping: cleaning living areas, making beds, dishes, laundry, taking out garbage.
• Monitor and redirect client behaviors to ensure safety.
• Meal planning and preparation.
• Medications: ordering, administration and monitoring.
• Arrange client transportation.
• Assist clients in getting around and participating in the community.
• Assist clients in managing their finances.

Minn Care Home Health is looking for Direct Support Professionals for our Assisted Living in New Hope, MN:

• Every other Thursday, Friday and Saturday (10pm - 7am)

• Starting at $18/hr

Minn Care Home Health provides home care services to adults with developmental and mental disabilities, and those in need of medical care after hospital discharge. The Direct Support Professional will perform direct client cares ordered by the client's physician and as documented in the client's care plan.

MAIN RESPONSIBILITIES:

• A. Assist clients with personal care: dressing, grooming, bathing, eating, toileting, transferring, positioning, mobility.
• B. Housekeeping: cleaning all living areas, making the bed, dishes, laundry, taking out the garbage
• C. Monitor and re-direct client behaviors to ensure their safety.
• D. Meal planning and preparation.
• E. Medications: ordering, administration and monitoring.
• F. Arrange client transportation.
• G. Assist clients in getting around and participating in the community.
• H. Assist clients in managing their finances.
