Job summary

Minn Care Home Health is seeking a Direct Support Professional to support residents in our Assisted Living homes. You will implement care plans and assist with daily living activities in a compassionate, respectful environment.

Responsibilities include personal care, housekeeping, meal planning, medication monitoring, arranging transportation, and helping residents engage with the community. A DHS background check and reliable work ethic are required. Starting pay is $18/hr.

• Housekeeping: cleaning all living areas, making the bed, dishes, laundry, taking out the garbage.
• Monitor and re-direct client behaviors to ensure their safety.
• Meal planning and preparation.
• Medications: ordering, administration and monitoring.
• Arrange client transportation.
• Assist clients in getting around and participating in the community.
• Assist clients in managing their finances.

Minn Care Home Health provides home care services to adults with developmental and mental disabilities, and those in need of medical care after hospital discharge. The Direct Support Professional will perform direct client cares ordered by the client's physician and as documented in the client's care plan.

MAIN RESPONSIBILITIES:
