Job summary

Easterseals PORT Health is seeking a part-time Direct Support Professional to provide in-home and community-based care for clients with IDD or MH challenges in Rocky Mount, NC. You will assist with daily living tasks, help maintain routines, and document progress in the health system of record.

Responsibilities include supporting habilitative activities, personal care, transportation as needed, and collaboration with a multidisciplinary team to ensure person-centered planning and safety.

Qualifications

• 1 year of direct care experience, personal care and/or CNA experience, preferred
• Must complete and maintain our training curriculum, including CPR/First Aid, Medication Administration, core competencies and other trainings

Responsibilities

• Provide care in the home of the individual and the community they live in
• Assist with daily living tasks such as housekeeping, meal preparation, cooking, and cleaning; personal care such as bathing and dressing; and employment support
• You may transport the client to appointments and run errands
• Help plan and organize their schedule
• Daily documentation and reporting in the electronic health system of record
