Job summary

Center for Developmental Disabilities, Inc. is seeking a dedicated professional to provide direct supervision and support for individuals in Day Habilitation and Pre-Vocational services.

You will implement and document progress toward clinical goals while ensuring safety during site and community activities. The role requires a valid NYS driver's license, solid communication skills, and the ability to lift up to 50 lbs.

Qualifications

  • High School Diploma or GED required.
  • Valid NYS Driver's License required.
  • Good written and/or verbal communication skills.
  • Experience with individuals with intellectual and/or developmental disabilities preferred.
  • Flexible work schedule to meet program needs.
  • Physical ability to perform tasks including lifting up to 50 lbs.

Responsibilities

  • Follows program activities schedule as set by the Program Supervisor.
  • Provides direct supervision and instruction to assigned caseload during all site based and community-based activities.
  • Implements and documents outcomes of each individuals program and clinical goals.
  • Assures safety of the individuals during all site and community-based activities, including protection from neglect/abuse.
  • Cooperates with program management, clinical, and support staff to ensure skillful implementation of Day Habilitation and Pre-Vocational Services.
  • Provides individuals with necessary training, support, and opportunities to achieve outcomes.
  • Trains individuals to interact appropriately with peers, staff, and community members, and to perform volunteer and work assessment tasks to staff satisfaction.
  • Maintains training in maintenance and cleanliness of the facility and safety of equipment for individuals.
  • Acts as a liaison for the Center and the individuals with the community.
  • Adheres to Center policies; completes required forms including time off requests, incident reports, and ABC sheets; uses electronic record keeping and time clock systems.
  • Builds and maintains positive working relationships with individuals, families, advocates, staff, and community members.
  • Demonstrates good judgement, seeks guidance when needed, and communicates effectively across parties.
  • Administers medication to individuals as per Doctors orders.
  • Maintains Centers driver eligibility and drives agency vehicles to access community-based services.
  • Attends meetings and training seminars deemed appropriate by program management.

Some Job Responsibilities

  • Trains individuals to interact appropriately with peers, staff, and with persons in the community, and to perform volunteer and work assessment job tasks to the satisfaction of the staff at the community site.
  • Provides the individuals with the necessary training in the area of maintenance and cleanliness of the facilitys work areas and the safe use and care of facilitys equipment.
  • Acts as a liaison for the Center and the individuals with the community.
  • Adheres to all Center policies; complete all Center forms as required including but not limited to: time off requests, incident reports, non-reportable, ABC sheets; utilized the Centers electronic record keeping system and time clock system correctly.
  • Builds and maintains positive working relationships with individuals receiving Day Habilitation, Pre-Vocational Services, their families and advocates, co-workers, and community members.
  • Demonstrates good judgement in the performance duties, seeks advice from program management when needed. Utilizes good communication and interpersonal skills with all parties.
  • Administers medication to individuals as per their Doctors order.
  • Maintains Centers driver eligibility and drives agency vehicles to access community-based services.
  • Attends all meetings and training seminars as deemed appropriate by program management.
Job Qualifications
  • High School Diploma/GED;
  • Valid NYS Drivers License required;
  • Good written and/or verbal communication skills;
  • Experience in instructing individuals with intellectual and/or developmental disabilities during real life experiences preferred;
  • Maintain a flexible work schedule to meet needs of individuals and programs;
  • Must be able to perform physical activities such as (but not limited to); twisting, bending, crouching, pulling, pushing.
  • Must be able to sit, walk, drive, and stand for prolonged periods of time;
  • Must be able to lift 50 lbs.
