The role includes assisting with daily activities, transportation, and participating in recreational outings while maintaining care records and communicating changes to the team. Qualifications

• 1+ year of experience as a direct support professional or similar role.
• Strong communication and problem-solving skills.
• Ability to multitask and handle a high-volume workload.
• Ability to work collaboratively with other team members.
• Valid driver6s license and reliable transportation are a plus.

Responsibilities

• Assist clients with daily activities such as bathing, dressing, and grooming.
• Provide transportation to appointments and other activities as needed.
• Participate in recreational activities with clients, including outings and community events.
• Maintain accurate and up-to-date records of care provided and report any changes in the client6s condition to the appropriate team members.
• Collaborate with other team members to ensure continuity of care and services.
• Other duties as assigned.
