Job summary

Waypoint seeks a Direct Support Professional-Community Supports and Membership to help members engage with their community, build relationships and pursue meaningful activities while promoting self-determination and independence. You will model respectful interactions and work within a team to provide safe, confidential support aligned with each member's plan.

Qualifications

• High school diploma or GED required.
• Must obtain Direct Support Professional Certification within six months of hire.
• CRMA certification as needed.

Responsibilities

• Cultivate a safe and supportive relationship with members and understand service plans.
• Provide individualized support to promote independence and community engagement.
• Assist participants in developing meaningful activity schedules to foster growth and inclusion.
• Follow Service Implementation Plans and care plans per Person-Centered Service Plans.
• Support participation in community activities, including providing or accessing transportation.
• Maintain accurate documentation and comply with licensing and privacy policies.
