Job summary

Qualifications

• Excellent communication, listening, and motivational skills.
• Ability to work in an interdisciplinary team and maintain confidentiality.
• English reading/writing proficiency and the ability to document accurately.

Responsibilities

• Provide services per Staff Action Plan and individual Safeguards.
• Document habilitation services per requirements (agency, OPWDD, Medicaid).
• Participate in Staff Action Plan development and Life Plan with the person, family, and Care Coordinator.
• Monitor safety and welfare; take corrective actions and notify supervisor.
• Provide supervision as stated in plans and safeguards.
• Encourage community integration and autonomy for individuals served.
• Offer training and encouragement for personal health care needs.
• Communicate health concerns to supervisor per protocol.
• Maintain required paperwork and documentation daily, including notes and reports.
• Coordinate transportation and vehicle safety; maintain driving certifications.
