Job summary

Youth Advocate Programs, Inc. (YAP) is seeking a Direct Support Professional to provide compassionate, person-centered care for adults with developmental disabilities in home or community settings.

This hourly, part-time role offers flexible hours and opportunities to support individuals in daily living and social activities. Responsibilities include hygiene and meal assistance, medication reminders, documentation in EHR systems, and collaboration with the care team to ensure health and safety.

Qualifications

• High School diploma or equivalent required.
• Experience with individuals with developmental disabilities preferred.
• CPR/First Aid certification or willingness to obtain.
• Proficient computer skills; experience with an EHR is a plus.

Responsibilities

• Provide person-centered care in home or community settings.
• Assist with daily living activities including hygiene and meals.
• Give medication reminders and monitor health needs.
• Support individuals in achieving personal goals and social activities.
• Maintain accurate documentation and communicate with care team.
• Ensure a safe, clean, and supportive environment.

Location: Titusville PA

Status: Part-Time Hourly

FLSA classification: Non-Exempt

Schedule: Flexible hours - possible for up to 40 hours per week.

Summary of Position: The Direct Support Professional is an hourly, part-time role dedicated to making a positive impact on the lives of adults with developmental disabilities.

• Provide compassionate, person-centered care in home or community settings, ensuring the health and well-being of individuals.
• Assist with daily living activities: hygiene, meals, and medication reminders.
• Support individuals in achieving personal goals and participating in social activities.
• Maintain accurate documentation and communicate effectively with the care team.
• Ensure a safe, clean, and supportive environment.

Qualifications/Requirements:

• High School diploma or equivalent is required.
• Additional coursework or certification in human services or related fields is a plus.
• Previous experience working with individuals with developmental disabilities is preferred.
• Strong interpersonal and communication skills
• Ability to work independently or as part of a team.
• CPR/First Aid certification or willing to obtain
• Proficient computer skills required; experience using an electronic health record (EHR) system is a plus.
• Reliable transportation, valid driver's license, and current auto insurance coverage is required.
• Bilingual (English/Spanish) is a plus.
