Job summary

ComForCare Home Health Care (North San Diego) is seeking a Direct Support Professional in San Diego, CA to provide compassionate, in-home care and support clients with daily living activities. You\'ll work under established care plans to enhance comfort and independence in the client\'s own home.

Qualifications

• Previous Direct Support Professional or caregiver experience is preferred.
• Compassion, patience, and dedication to supporting individuals.
• Ability to pass a background check and drug screening.
• Valid driver\'s license and reliable transportation.
• Strong communication and interpersonal skills.
• Ability to follow detailed care plans and work independently.
• CPR/First Aid certification (or willingness to obtain).
• High school diploma or GED required.

Responsibilities

• Provide hands-on personal care including bathing, grooming, dressing and hygiene support.
• Assist clients with daily living activities to promote independence and dignity.
• Offer companionship and emotional support to clients and families.
• Assist with meal preparation, light housekeeping, and errands.
• Monitor and document changes in client health or condition and report to supervisors.
• Safely assist with mobility, transfers, and ambulation as needed.
• Administer medication reminders in accordance with care plans.
• Follow individualized care plans to meet each client\'s needs.
