ComForCare Home Health Care is seeking a compassionate Client Care Coordinator for our North San Diego team in San Diego, CA. You will be the key link between clients, families, and caregiving staff to ensure high-quality home health services are delivered with empathy and professionalism.

In this role, you will assess client needs, develop personalized care plans, schedule and match caregivers, and monitor satisfaction while maintaining accurate records and complying with regulations.

Qualifications

• Experience in home health care or related care coordination.
• Strong interpersonal and client-focused communication.
• Excellent organization and multitasking in a fast-paced setting.
• Familiarity with scheduling software and basic computer applications.
• Knowledge of home care regulations is a plus.
• Empathetic, dependable team player.
• HS diploma required; associate/bachelor preferred.
